The only two things I can think would cause you problems.
in extension settings - flash mode during co conversation should be EFA (it is by default)
second could be the flash time set in co card property.

thanks I have set the flash timer at every setting and tested but still does not work, when you use the flash key nothing happens you don't even here it change the call or click tried using the hook switch but it just hangs up the call no matter how quick you touch it. looking at the programming can not find a place to turn off EFA so I would presume it is still set, also tried different sets.
May be you should check Off premise Extension "OPX" in the feature manual.
It could offer more guidance.
