Issue with H323 Trunks (non-SCN)

I've seen this on a few installs and the systems have varied between 5.0 and 7.0. The issue is that calls across an H323 non-SCN line will go thru on the first call with ringing and audio and all will work fine. Hang up, make a second call and the other end rings but the is no audio, the other end has answered. But here is where is gets extra strange, exactly at two minutes from the hangup of the previous call, the audio kicks in on the current call that has no audio. I have used Wireshark to watch the packets but cannot find the cause. This has happened using Netgear to Netgear and Cisco to Cisco small business routers.

On my test bench I have connected 2 systems without VPN and have not experienced the issue. My guess is it has something to do with AVRIP packets keeping things happy when using SCN over VPN, but no such packets when H323 only. I've tried variations with QSIG and H450, no help.

To me this is similar to an issue I had with SIP trunks and ADTRAN router's ARP request killing audio for a while but can't make the connection to this issue. Any opinions appreciated.
 
It is a timeout so network related for sure.
