Here's a start
1. Create a table with ID, Name and Notes.
View | Table | More Tables
Click on Entry; Click on Copy
In the Table Definition popup dialog
a. Change the name to ABC
b. Delete all lines except ID and Name.
c. Insert a line for the Notes field.
d. Click on OK
e. Click on Apply.
2. Create the report
View | Reports | Custom and click on the Select button
Look for the Task report, click once on it and click on the Copy... button.
In the popup dialog,
a. Click on the Definition tab
b. Change the Name to DEF
c. Change the Table to ABC
d. Click on the Details tab
e. Put a checkmark in the Notes box
f. Click on OK
This will take you back to an earlier popup. Click on Preview.
