Is a new license and authentication file needed when upgrading from CM4.0.1 to CM4.0.3? Thanks.

Nope. Just perform the upgrade. This is called the dot release upgrade, that doesnt require any license. If you push the upgrade in CM5 (ONE LEVEL HIGHER THAN YOU HAVE) requires a new license.
