IPoffice 500 DS30 Mod Blow up

Have an IP office 500 with a DS30 mod. The DS30 Mod has blown out twice now. We now have put in a third DS30 mod. It is about a month or so between blow outs. The firmware matches the software on the main unit. When the mod goes all the phones go dead, reboots don't work, and manager does not recognize the module. The module powers up, with a red light. We do not think this is related to electrical storms. Any other thoughts?

Grounding and out buildings and really crappy cabling. Use the knowledgebase to establish proper grounding and IROBs if necessary. Perhaps consulting an electricial to be sure that the building is grounded properly.
 
have you changed power supplies? have you tried to power it up with no phones plugged into it?

Definitly a problem with the cabling
Did there perhaps was an old system with S0 phone's ?
The S0 phones needed resistors to work
Those resistors keep building up power untill it is too much and then blows up the module

im with Kevin. when you swapped the modules did you swap the power supplies?

I had the same issue earlier this year. We replaced the DS30 3 times in a week right after installation. Everything was grounded (IP500, surge, and rack).

We swapped the power supply and upgraded to a newer firmware and the issue was resolved. I figured it was the power supply since the other DS30 had no issues ever.
 
Sorry for not updating. I waiting for the latest bad DS30 to ship back. When it does I will fire it up and see what happens with no phones attached and with a power supply change. Will have to find out if tech that put new one in used the new power supply. Yikes!!!
 
AVAYA does not ship a new power supply on a RMA.
