IPO Module won't link up

I've had a customer where we installed an IP Office 406 v2 and 3 modules that were a DIG30, Analog 30 and a POTS 16. A month or so after install, the Analog 30 and POTS 16 modules lost it's link to the controller. Tried rebooting several times but could not restore the link. It would constantly blink. I've swapped out the modules to restore the customer back to service but I was wondering if anyone knew why these modules behave this way or if there's another fix to restore the link to them. Any input would be MUCH appreciated. Thanks.
 
often applying the firmware to them fixes them.

you might need to DTE it first and erase the bin file off it, and allow it to get a fresh bin off the IPO
 
how did you do the reboot?

un plug all modules an ipo, then plug back in the modules first wait until the're green then at last power up the IPO itself.
