IP Office Module won't link

I've had a customer where we installed an IP Office 406 v2 and 3 modules that were a DIG30, Analog 30 and a POTS 16. A month or so after install, the Analog 30 and POTS 16 modules lost it's link to the controller. Tried rebooting several times but could not restore the link. It would constantly blink. I've swapped out the modules to restore the customer back to service but I was wondering if anyone knew why these modules behave this way or if there's another fix to restore the link to them. Any input would be MUCH appreciated. Thanks.
 
First of all when you rebooted the expansion modules did you make sure that you started the modules before you started the control unit?

Try reloading the firmware to them using the DTE port!

Also There is an excellent ip Office forum within tek-tips you would be better off posting in.

I have the same problem - broken firmware on the IPO Analog30 expansion module.
