I am not an expert in this area, but I will try to help.
I assume that you have a Comcast box (Arris?) with the cable wire going in, and analogue lines coming out, going into an ATM4 on the IPO.
I've had different issues with Comcast lines on an IPO500V2. I'm not familiar with the IPO 406, but you may want to try the following:
1) Make sure the tip and ring are correctly wired for each of the analogue lines coming from the Comcast box to the IPO.
2) In System Manager, under Lines, check the Analogue Options for each of the lines. If you have an option for "Automatic Impedance Balance Match", run it for all of the lines (run it for line 1, and you should be prompted to run it for the other lines). If you don't have that option, try a different impedance setting (in my area, IPO required an impedance setting of 600 to work with the Comcast lines). I have also found that I needed to change the Disconnect Clear to 750ms.
