Is this a PBX to Public Switch scenario, or PBX to PBX?
If it is PBX to Public Switch the most likely cause would be that the protocol (switch type and variant) are not set correctly on your equipment.
If it is PBX to PBX then the most likely is that one PBX or the other isn't fully comaptible with the relevant standard. But in this case make sure the swithc type is configured correctly too.
Most likely you'll need to capture a Layer 2 + Layer 3 protocol capture and send it to the equipemnt vendor. Post it here and I'll be glad to look it over.
