I have an ISP hosting my Email address in which this ISP forwards all email to my internal exchange server. Would setting up an Anti-Spam server on my exchange server resolve the spam issues. Seems obvious that it would so any comments or suggestions are appreciated.

Yes I believe it would. You could also use an external Spam Solution like Barracuda Networks. It's a box you purchase. You have your ISP forward to it and it forwards to you. Then the Pam never reaches your Mail Server and you don't have to worry about extra software on the mail server.

I'll second the idea for a gateway approach. I have SpamAssassin running on the gateway and it blocks over 95% of the spam. Plus, the gateway mail server automatically maintains a list of valid internal addresses so the Exchange server doesn't get bogged down handling "dictionary spam".
