Index Corruption

Does anyone have a link describing some of the main events/issues that would cause an index to become corrupt.

I am very familiar on how to check for these errors as well as how to correct them (DBCC commands, Drop/Rebuild indexes ...)

What I am looking for is possible reasons this may occur besides the general issues of Hardware and/or Software.
