What OS is on the client, and how is it connecting to the server? What kind of connection do you have on both ends?
Most common cause would be failure of the initial GRE protocol to get from the server back to the client. Some NAT devices on the client side consistently cause the problem, particually older models. Also make sure that your server has a route to the client, do a ping from the server to the client prior to trying the VPN connection.
