You can try and find the shareware you installed that came with the spyware and remove it... OR
You can get a program like AdAware and install it. It will scout threw your system and take out any known SpyWares.

This looks like a custom trojan. AdAware and such are like virus scanners. They only recognize well known spyware.
Get the help of an expert, fast. Especially if your computer contains anything valuable, or is connected to systems that contain anything valuable.
If you can't get help, I recommend formatting the driver and rebuilding. Don't mess around...
