you can register online in

http://www.prometric.com.

There you can find information about testing centers and exam fees.

I advise you to contact first with the testing center that fits you and check with them for availability. Then you can register for the exam in Prometric or ask them to do it for you.

To prepare the exams i read the courses pointed in the certifications track:
- Fundamentals of Unix
- HP-UX Admin I
- HP-UX Admin II

Recommended book for reference: HP Certified, by Rafeeq Ur Rehman, ISBN 0-13-018374-1.
