You need to make a form that has the comboboxes and have a query that references the controls (comboboxes) on the form.

I did not look at you attached files. But then again, the links are invalid, so I couldn't look even if I tried (They reference a local file, they should be a URL i.e. You can always just paste the text from the sql statements.
One last thing: Sometimes I wonder; "Is that someone's signature? Or do they type that at the end of each post?
