How to tie checkboxes on a from to specific items in a table

I am having trouble setting checkbox's on a form to specific entries in a table. The name of the table is States and contains the following 4 fields. ST (the Abreiviation) , Full Name, Region (Single letter to represent which of our offices handles the state) and Selected (Yes/No). There are 53 entries in the table counting the states and territories.
The form is to be a selector for which states to include for a custom directory that goes to our clients.
I have created a form with all of the checkbox's and they have labels to specify which state they are for. How do I tie the checkbox on the form that is named "AL" to the selected field of the Alabama entry in the table.

Why not simply a continuous form bound to the States table ?

I was able to make a continuous form using the wizard, but was hoping to make a form that showed all of the entries using multiple columns and didn't have to be scrolled.
