How to set up page for a BookCover in PAGEMAKER

What do I do to set up my work page for a Book Cover?
I am not sure how to set it up for the spine and is the book cover a larger width and length than the text pages would be.
Just some helpful hints on setting up the page before I import a picture would be greatly appreciated! :) [sig][/sig]
 
Assumed 100 pages (pieces of paper) inside
Assumed thickness 0.1mm each
Assumed paper size 200 x 300 mm
Paper thickness 10mm
Case a)
Cover is cut together with pages after offset printing
Assumed cover size 200 x 300 mm
Assumed cover thickness 1mm each
Define total page size ( 10+200+2+10+200+10) x (10+300+10) mm = 432 x 320 mm
Define printable area ( 5 +200+2+10+200+5) x ( 5 +300+5) mm = 422 x 310 mm
Print image in printible area (loss about 5mm at each edge)
Keep distance from border with text with respect to inaccuracies
After cutting the size (top view) is 200 x 300 mm
Case b)
Thick oversized cover with image going from front to back side of front cover and rear cover (resp)
Give your files to offset printing company
They will assemble with professional experience
