how to send/recieve modified outlook form

I am in a phone message form (phone icon in top left corner of outloook xp). I click design form i make modification changing return call to returned call in label. I then publish form in my personal form directory as Phony Message. Then I open form it looks great. then i send form to myself and I don't see my changes, i instead see phone message form not my new 'phony message form'.

I had checked save definition in item. please can anyone help? what am i doing wrong.
