How to Create Hyperlink in PDF doc

I have a newsletter that I'm creating in QuarkXPress, then copying it to an Adobe PDF format. I want to include hypertext links in my Newsletter, so ultimately, when a user opens the PDF file, they can click on "links" (which will be links to websites (not links within the document).

Can someone explain "in laymen's terms" how to do this? I've read through all the help files, and there's no clear explanation. I've done everything they say, but it's confusing me, and not working. I think I'm hung up at the "destination" part, if that means anything. It said to "scan your destination" (or something similar to that), and I couldn't go any further because I couldn't find where or what I was supposed to "scan?".
