hoping for simple answer

my client wishes to have a template for emails to have 5 checkboxes sorta like a fax coverletter detailing what is attached in the email. they use Outlook 2000 but this form will be sent to their clients (not on the network or exchange server) we will have no idea what email program will be receiving this email. what would be the best way to accomplish this?

When creating the form you will need to elect the option under the PROERTIES tab of the form to Send form defintion with item . But if you don't know what program they are using there is no way to be 100% certain that they will see the form correctly. joegz
