HELP!! Using Form Values in golive

I am trying to get the information from a form textbox to go to a specific directory on the same server ...
in other words.. if the user on

http://www.brennerlennon.com/webchecktest.html

enters "xxx" in the form and hits submit
then the new page to open up should be

http://www.brennerlennon.com/webcheck/xxx

I have gotten close, but the new address comes up like this:

http://www.brennerlennon.com/webcheck/?=xxx

PLEASE HELP!! - ANY SUGGESTIONS???
