[Headings] in Outlook Mail Subject Line

How does the
in brackets show up in the Outlook subject line? When I get an email from my ISP, it always starts the subject with [Speakeasy]. Suddenly, though, I am getting emails from trusted sources that say [Spam] in the subject line. I can't see that it is generated from Outlook, or Spamnet. Where is this coming from? Can anyone help?
