Storage Processors A & B (failover mode=3)
PowerPath 53 HF01
VIOS 2.1.0.10 FP 20.1
Internal SAS Drive 146G (mirrored to SAN)
SAN OS mirror LUN (w/reserve lock for partition mobility)
2-Fibre Cards Qlogic (one of 2 on a combo card)
cmd_num_elems=200,max_xfer_size=0x100000,fc_err_recov=fast_fail;dyntrk=yes
2-20 port Brocade switch modules for Blade Center (4G)
rootvg (quorum disabled)

When zoning for Storage Processor A is removed on the switch, PowerPath does not fail over paths to B with new ownership of B.

This is for the mirrored OS LUN which falls into a "missing" state for the hdisk (lvs are marked stale).

powermt display=both HBAs degraded w/two of four paths dead

Mirror is restored only via a reboot.

errpt:
backup path inoperative
connection failure
FCP_ERR6 (DC73C03A)-sense data 000 0003
pv declared missing
i/o error detected by lvm
disk op error
failed to write volume group status area-sense data 0001 3E8A
mirror write cache write failed

Regardless of SP failure, the OS mirror LUN drops IO.

Tried:
removing disk reserve, fast_fail, dyntrk
