Frustrating Intermittent Problem - Call Transfer Issue between IPO

Hello gurus,

I am having a very frustrating problem. It occurs intermittently.
Setup is like this:

PSTN/GSM [Analog Card/GSM gateway] -> IPO406v2 (5.0.15) <-> IPO406v1 (3.2.69)
*There are IP trunks between these two IPOs (but not in SCN), and also between GSM gateways and IPOs.

What happens is that sometimes (and only sometimes)a call comes in from the PSTN (I have analog card)or GSM network, goes to the Operator connected to IPO406v1. She transfers the call to another operator connected to IPO406v1. When the problem manifests itself, the destination extn does not ring! A green light shows on it, but that operator is not able to pick even when they press the button to pick.

In another manifestation of the problem, I have a GSM unit which is configured to direct calls to an AA (using VM Pro).
A caller calls a number, keys in *EXTN and the call gets forwarded to the extn specified. Now, this works until something weird happens on the IPO406v1 when calls will get forwarded (to any extn) but there will be no ring on the destination extension. The light will show, but call cannot be picked up.

So in general, it looks like there happens a call transfer problem between the IPOs, because even the AA does it's transfer via the IPO406v2, right? What causes this is what I cannot figure out. This problem is only about external calls being transferred. Internal calls are fine all the time, even when this problem occurs.

The IPO406v1 also has a GSM gateway connected directly to it on the same LAN subnet. The same problem also occurs sometimes when calls land on that GSM gateway - calls fail to ring on operator extn, light shows but call cannot be picked.

This is quite frustrating. It appears to me that something odd happens on the IPO406v1. I just don't know where to look any more! I don't know what to think:-(

Any insights are welcome.
