It looks like the best source of information is the documentation at
I recently upgraded a FreeBSD 5.3 server to FreeBSD 5.4 simply by running Sysinstall from the FreeBSD 5.4 CD with no issues. If you plan to upgrade from FreeBSD 4.x, I wouldn't suggest this path, but be sure to read the documentation at the link above.
