Free survey script suggestions?

Hello,
I am looking for a free survey script, I just have a few fields I need people to vote on and the rest are text boxes for them to fill in info.

Any suggestions on a script I can run on a Windows 2k box?

That is assuming you want to use IIS. What webserver are you using?

IIS is the webserver.

I checked out some of those places but I find a lot of "mail" forms, I am not sure if that is what I am looking for.
I don't want it to mail it anywhere, just write it to a file..?

Do you know of a free script that will do that? or a name of one I can search for?
