FORMS - CGI-BIN - MAKE IT WORK

I have a problem trying to get a form to work and for some how, when you click the submit, it just go to an error page.
I'm trying a lot of differents ways and still can get it to work. Doed anybody has any idea or what to check up.?

What type of page are you calling? Is it an ASP or is a PERL script or something similar?
