formmail.pl shows () in the recipients from part of inbox I want it

when someone hits submit on our company website forms the recipient who the mail form goes to receives a () in their inbox in the from column and I want it to say "REQUEST FOR INFORMATION"
What do I need to change in the script to make this happen?

If you're using an old formmail.pl, please stop, and get the secure version from NMS project. Insecure formmail.pl scripts are being used by spammers and can ensure your company mail server becomes an open relay, which may get you blacklisted.

The NMS project provides excellent support to anyone using the scripts, and its all free. The scripts have been rewritten with security in mind, and even Matt himself now recommends the use of NMS scripts.
