Form to Search 4 Tables

This is what I have as a base structure: I have 4 tables all related that have an id, name, and then the 12 months of the year on them. I want to create a form has the user enter an id number, a name, and then one of the 12 months and then display the information of all four tables on a subform perhaps. I'm an amature when it comes to VBA coding but any help would be amazing. Thanks!

as your database schema seems far to be normalized.
