/exchange/forms/IPM.NotefrmRoot.asp? Url Creation

I am experiencing this well documented issue when trying to read
an email in outlook web access. The above url should read
/exchange/forms/IPM/Note/frmRoot.asp?

System

My cdohtml.dll version is 5.5.2655.77, the latest. I have installed and tested >Psp1cdoi.exe
(sp1) fix for this, no luck, reverted to most recent.

I have manulally unchecked the "Archive" flags from each and every file and folder
in the system, including the logs.

The exchange filters in iis prevent the system form working at all, so I
have left them out of the ISAPI filters in IIS.

Urlscan is not the issue, since I can edit the url and reach the mail.

Everything else works perfectly.
