Excel button disappeared; can't add new button

I have a protected spreadsheet (Excel version 2000) with a form button. The user reported that his button had disappeared. I opened the file, and sure enough, no button. I unprotected the sheet, and tried to add a new button using the forms toolbar. The whole toolbar is grayed out. Other than protection, what would prevent the forms toolbar from being active?

I also tried cutting and pasting a button from another workbook, but Excel won't let me paste into the problem workbook.

Is the STRUCTURE of the workBOOK protected ??

All protection was disabled. This is really screwy. I just opended the file again and the buttons were back. All I can think is that it was some sort of network file access problem or maybe the file was opened as a different Excel version.
