Every other call works

We have a Nortel connected to an Avaya 8730. I have a Distance Steering Code set up to send DN 4914 from the Nortel to the Avaya through an audiocodes box. Every other call works fine. The calls that fail do so on the same channel and T as the ones that succeed. I ran the command...

I am looking for guidance. Is this something in the Nortel? What could it be? or in the audiocodes box? Someone traced it to the audiocodes box and claims the failed call never hits it.
 
I would start looking at the history file in the Nortel. Start simple, could be a cabling issue. card issue, physical layer first. I would then start looking at your configs and posting them here. Obviously this is a PRI so who is the master? How is your clocking set up? Are you taking any errors on the circuit. When you say "Nortel", that could mean a lot of different style switches, 1000e, 1000m, what type of card are you using? TMDI, Dual Port PRI, what type of d-channel board. Lots of variables here.
 
Cause No. 41 - temporary failure.

This cause indicates that the network is not functioning correctly and that the condition is not likely to last a long period of time; e.g., the user may wish to try another call attempt almost immediately.

What it means:
This means that there is a temporary failure at the physical layer on the ISDN network. If you remove the ISDN cable from the Netopia, you would see this. It's usually temporary.
