Embed word into outlook form

How do I go about using a word document template as an Outlook form?

I am designing a form for that will be used by an end user to request a change in our system.
I want to be able to create a Word template that will have a standard set of questions for the user to fill in. This template will be held in an Outlook Task folder for the developers to access.
