Echo on 5410 set? - 3 different systems 3.1.65 & 3.2.55

Anyone had this issue?
I have 3 different sites that we have 1 or 2 5410 sets that the caller is getting an echo when they dial out. all 3 IPO's are 406v2 and the ext's are not tried to any particular module. It is a quick echo, not latent and it mostly happens on outgoing calls. The are going out of a PRI in all cases and we have already replaced the phone at 2 of the sites.
 
On the phone itself, go into "Options", on the second page of buttons is where you will find the AGC settings for the handset, headset and speaker. Good luck.
 
I have seen (am seeing) this exact same problem. I have yet to figure out what is causing it. 54xx phones with a PRI seem to be the common set of variables. Dropping the db level, adjusting the "LongHaul" setting to -7.5db and -15 db, adjusting AGC on the set, nothing seems to help...If you find anything out, please share. Of course, I'll chime in if I find anything out. Z
