E1 R2 Lines stuck in "clearing" state

Hello guys I need some help with this issue, I have an IPO v5.0(8) that randomly gets line stuck in clearing, any idea how can I know why this would be happening? the IPO has a VMPRO with an AA answering all the calls
it works good to the point where the lines get stuck and I have to reboot the IPO in order to clear them.
any help is greatly appreciated
