e-mail Form with File Attachment

Hi Experts:

I have a web page that has a form with few fields and a field for attaching a document. I would like to receive this information thru e-mail when user clicks on the submit button. I am able to recive the data for all the fields that users have filled but I am not receiving the file attached with it. What kind of code do I need to write to receive the attached document with the e-mail?

Any help provided would be appreciated.
