DTI card all lights flashing

Voice service went down on an MICS using a T1 circuit (not a PRI). The provider is claiming that the circuit is testing clean, although based on past experience with them I'm not convinced that the problem is not on their end still. I'm not on site to get into the system management but I had someone cycle the power on the unit. On power up, the DTI card appears to be stuck in the initialization state with all LEDs blinking. My question is, could a provider side issue cause this behavior where the DTI just blinks all LEDs continuously and never exits the initialization state? Or would this seem to indicate that there is a hardware issue with the MICS?
 
If this was working place a loopback before the smart jack and see if your T1 card settles down, if it doesn't change it

If it's an old MICS it might have lost it's database during the power recycle. All the lights flashing on the DTI card sounds like it's defaulted and reset all the trunk cards to analog. Ask the customer if the date defaulted to Jan. 1. If not, I'd start with a loopback plug as suggested by Perry, and then move on to the DTI and services cards if the DTI won't sync.

Got some more info... no date is even being displayed on the phones. The red VM lights are flashing and all of the button indicators are flashing. So it seems to be stuck in startup. Has the cabinet failed?
