Diskadd gives error in UNixware 7.1.1

We installed unixware 7.1.1 on our Hp Netserver LH4 having in built RAID controller. There are 3*9.1 Gb hard disks running under RAID 5 attached to this RAID controller. Also the bootable hard disk is another 9.1 GB attached to adaptec SCSI controller.
The server boots okay, mounts filesystems in boot disk properly.
When we try to add the RAID drive using "diskadd" command, it takes all inputs and reaches upto the message:-

Following are the allocations
xxxxx
xxxxxx
xxxxxx
Is it acceptable to you(y/n)y

on pressing "y" we get following error :-

UX:sh (sh): ERROR: disk3: Not found


However the process of creating slices/filesystems continues and completes. The new filesystems get mounted properly.

But what is this disk3 error and why does it appear ?

Can anyone help ?
