Is that on the form with blank tone names, cadence step and tone (frequency/level) or is it in the tones & conference loss plans form? I enabled (y) the 440Hz Secondary-dial Tone in the first page but to no avail.

What will I place on the other fields (cadence step/frequency)? What frequency can I use? Is it service affecting if I made something up?
