Rizunb,
you should be able to select the query that you want to base the form on, click Insert on the menu bar, then select Form. From there, you can try Form Wizard or Autoform: Tabular or Autofrom: Data Sheet. You should be able to play around with those to find something to your liking. I hope that helps.
