Database Login Prompt

When logging into a database, the application automatically brings up the prompt for a user name and password. How do I setup a database login prompt to close the application if the cancel button is pushed or to many failed attemps?

recode the application to count the number of login tries and once exceeded exit.

What database are you using? Did you code the login screen or is it built into the package?
