data in field is also text box header

when I take and build a form using a quiery i wrote you have the column heading and then there should be a part number under the heading. example

36 gb 15k HD - text label
310-0001 this is the part number that should be appearing in the text box from data generated from the quiery insted the data from the text label is appearing like the following example
36 gb 15k HD - text label
36 gb 15k HD - tex box data
this is what appears when i choose to build the form using the query, but when i go back to query view the part number is displayed under the correct field. do you have any idea what may be causing this.

simple the tile of the label is appearing in the data field insted of the acutal data which should be the part number
