Data entry forms in Access project 1

I have a standalone MSA database that i'm converting to a ms project that communicates with an SQL server. But whenever i have a form with 'data entry = true' it shows up completly blank. not controls or anything. how would i go about fixing this but still allowing the user to use data entry forms?

I believe data entry = true indicates that the user may only add new records. Setting it to false indicates that the user can edit existing records and add new records.
