It is, if you include the form in the Notes mail template. You'll need Designer access to the mail template, the Notes Designer client, and at least a basic knowledge of Notes development.
Beware though, that any modification in your Notes mail template will not be available to anyone outside your organization since they will not have the special form. Also, although you can send the form with the design included, beware that not all users will be authorizing its use (I don't, for example) for security reasons - which means your modification is pretty much for internal use only.
