I have a table with the following

Plan Month Rx's Sales
Great West 1/1/05 1 $10
Great West 2/1/05 2 $15
CBC 1/1/05 3 $12
CBC 2/1/05 4 $13

How do I creat a cross tab so the data displays
1/1/05 2/1/05
Rx's Sales Rx's Sales
Great West 1 $10 2 $15
CBC 3 $12 4 $13
