Creating a List box for reports

Have several reports that I want to add to a form. How do I create a drop down list for those reports?
 
What do you mean by 'add a report to a form'?
The phrase means nothing to me, I'm afraid.
 
Sorry not enough information,
Have a form created for shipments. Currently have control buttons to inventory reports. want to create either a list drop down to pick what report, instead of having alot of control buttons.
