Creating a E-mail form (Template)

I am wanting to create a for using outlook which allows users to select various drop down menus etc to arrange themselves a shift swap, within a contact centre. Looking at the predefined schedule form, it would bear some resemblance.

I have never designed a template before & would like some info.

Does anyone know of any good websites or books that I can read. Alternitively if you have come accross this before point me in the right direction.
