CreateDialogIndirect and dialog box template 1

Hi,
Has anyone ever used the CreateDialogIndirect function to create a dialog? I am looking for some sample code, and have not been able to find any at all. I am more interested in how to construct the LPCDLGTEMPLATE structure. It is a "bit" complicated. If anyone has sample code to post or knows of a web resource or book, that would be great.

Download the example here. It's for an old version ( MFC / VC-4 ). But hope it helps a little
