Create TOC Utility problem

When I try to create a TOC in PM 6.5, I get an error message that says:

"Create table of contents internal error.

Internal error: Duplicate style name.

8213:5510"

I go to styles and clean out any "duplicate names" but it doesn't seem to help. What am I overlooking? [sig][/sig]
