create a history table

Hello,

I want to create a history table that stores records that have been changed in table "Project List"

On table Project list I want to use a Change Indicator that is set to "yes" when a command button "Change" is pressed and any record on the Project List is changed.

I am thinking that I need to create a query that will create a change/history table.

I do I structure the change table and query to be able to a 1 to many change/history table?
